This is miliary tuberculosis in an adult woman.  She was also sputum smear-positive for acid-fast bacilli, a usually rare finding in this type of tuberculosis. The reason for the sputum smear-positivity is found in the cavitary lesion in the right upper lobe, draining directly into a bronchus, clearly visualized by the air bronchogram.
