This patient with a massive pleural effusion had also sputum smears positive for acid-fast bacilli because he had simultaneously pulmonary tuberculosis (radiographically visible in the right upper lobe). Because the patient was dyspnoic at presentation, some fluid was drained. As a rule, not more than 1,000 mL should be drained because of the danger of hypovolemic shock.
