The strongest yet identified risk factor for the progression from tuberculous infection to tuberculosis is HIV infection. This should thus be an important group to target for preventive therapy. Furthermore, tuberculosis produces Tumor Necrosis Factor alpha, a factor that also stimulates HIV replication. It is argued that provision of preventive therapy may therefore postpone the onset of AIDS.
