Pericardial tuberculosis was a relatively rare manifestation in the past (with the exception of some populations). It is now seen with increased frequency in patients with HIV infection. The diagnosis is apparently difficult to make in the field. In this patient, the likelihood of a correct tuberculosis diagnosis was increased by the finding of pleural calcifications and axillary lymphadenopathy.
