Rifampicin has excellent penetration into critical tissues such as cavity lining, lung parenchyma and kidneys. Its penetration into the cerebrospinal fluid is, in contrast, much poorer, and problems thus may arise in patients with tuberculous meningitis caused by an isoniazid-resistant strain: isoniazid has excellent penetration into cerebrospinal fluid.
