Because treatment for tuberculosis is long, the number of medications to be taken regularly large and irregularity with treatment thus expected, directly observed treatment has been proposed to reduce the risk of acquiring drug resistance.

To truly assess efficacy in a clinical trial, it must be ensured that drugs are actually taken. In The Union's trial, all drugs were given directly observed during the intensive phase. In the continuation phase, one-month supplies were provided. In many countries, a large proportion of patients is actuall not directly observed neither in the intensive pahse (during which the risk of acquisition of drug resistance is particularly large) nor in the continuation phase.
