BCG is generally a well-tolerated vaccine causing rarely serious adverse events or complication. Different strains of BCG cause complications to a different extent.

Changing the production site (and thus the procedures) of the BCG vaccine led to a rapid increase in osteomyelitis caused by BCG in Sweden. The number of osteomyelitis cases became in fact more frequent than childhood tuberculosis, a clear indication that mass BCG vaccination was no more indicated in Sweden. Discontinuation did not lead to an increase in childhood tuberculosis.
