Contacts of newly identified cases with a positive tuberculin skin test are likely to have become infected in the recent past. Their risk of tuberculosis is considerably higher than that of tuberculin reactors of unknown or long duration.  In the Netherlands Navy a case among recruits occurred and infected a large number of other recruits.  This setting was used to test the efficacy of preventive therapy compared to placebo.  With 92 per cent protective efficacy it provided the highest attainable efficacy in all studies.  In two other studies among contacts protection was above 70 per cent, but in Japan the results were disappointing.
